It's form 2B and 2C, to be completed by you and also by your employer. Then I think there is a third form that's much simpler, and is to be submitted along with NYSC ID card on POP day (that's the one I think is the terminal letter, showing you've finished service so you get the discharge certificate).

In addition to online registration on NYSC website www.nysc.gov.ng OR www.nysc.org.ng all OOU graduating students who wish to participate in the NYSC 2014 BATCH ‘C ‘ Orientation are to visit OOU website www.oouagoiwoye.edu.ng/portal/students/ to complete the NYSC Pre-Mobilization form and print it for their personal record.The Registration had since commenced on Wednesday 3rd September, 2014 and shall end on Monday 15th September, 2014.Please comply.Signed Students Affairs Officer.

So true. When it's available in the various LG's the ZI or LI will pass the info across through either the CLO or the various CD presidents.
