Real Madrid manager Jose Mourinho and Atletico Madrid striker Radamel Falcao have reportedly agreed to sign for Chelsea next season.Mourinho, who managed Chelsea from 2004 to 2007 and still remains a favourite with the Stamford Bridge faithful, has long been linked with a move back to the club he led to two Premier League titles.

The Portuguese tactician, a close friend of Chelsea owner Roman Abramovich, led current club Real to the La Liga title last season, but Los Blancos have failed to reach the same heights this campaign, and it is expected that the 50-year-old will leave the club at the end of the season.

Mourinho's current contract with Real expires in 2016, but the Spanish outfit is believed to be interested in offering the former Porto manager a new deal over the summer in a bid to tempt him to stay.

However, according to German publication Bild, Mourinho's next destination in his illustrious career will see him make a return to Chelsea as Abramovich seeks to secure a suitable replacement for current interim manager Rafael Benitez.

The report goes on to claim that with current Chelsea strikers Fernando Torres and Demba Ba failing to live up to expectations, the Russian oligarch has also struck an agreement to bring highly-rated striker Falcao over to the club next season for a reported fee of €60million.

Falcao's signing is believed to be Abramovich's 'special welcome gift' for Mourinho, who has reportedly expressed his desire to work with the Columbian hitman.

The news could not have come at a worse time for Real, who were soundly beaten 4-1 by Borussia Dortmund in the first-leg of their Champions League semi-final tie.
