Doctor In The House:Obstetrics And Gynecology
« on: January 09, 2008, 02:21 AM »

Hello,
I am an ob/gyn in private practice in NY.  I will be willing to answer Ob/Gyn questions with the normal perfunctory disclaimer.  Thank you Dr. Balogun and Naijaking for this great idea.

4/21/2009
It has been over a year since I started on this thread.  I would like to thank all of you for allowing me the opportunity to help my own with the skills I have learnt in a foreign land.  You all have been more than gracious with your blessings, thanks, and prayers.  This being said, I am not here everyday.  This is work I do pro bono on my own time.  I am in a busy private practice and I try to answer questions as soon as possible.  Please refrain from follow up posts asking why I have not responded.  Thank you so so much
7/3/09
I thoroughly enjoy helping however, although I have asked politely, several posters are still repeating their posts.  It makes the thread longer than it needs to be.  Henceforth, I will not reply any posts that are repeated within a week.  Thank you and I look forward to helping you

7/17/09
I will be on hiatus for approximately two weeks.  My cousin, Naval commander Kolawole Awe was killed in the attack on the depot.

Dr. Kathy, u're powerfully welcome. Now let's start with the case below:

I am 8wks pregnant and I have  multiple intra-uternine fibroids and they are quite big. My GP said he can only 'manage' it and see how far it would go.Does any doctor in the house know what I can do to carry it to term?Or is there an alternative medicine I can use to at least suppress the growth of the fibroid and allow the fetus to grow, they are obviously competing for space and the poor fetus, though the legal occupant, is small against the foe. Somebody should please come to my rescue. It's my first (not really, Ive lost 2 previously) and am NOT very young.  Thanks.

@titilaelae
Fibroids are basically smooth muscle cells that respond to estrogen which is increased during pregnancy.  There is nothing you can do to decrease their growth in pregnancy however there are a couple of things that your ob can do.  1, they need to check your cervical length to make sure you are not dilating or thinning out prematurely.  If your cervix begins to shorten, you need to go on bedrest or perhaps get a cerclage which is a stitch to strengthen the cervix depending on the age of your baby.  2 They need to watch out for degeneration of the fibroids.  This happens when in response to the estrogen, the fibroids grow so large that they outgrow their blood supply and begin to die.  As they die, they release chemicals which can cause premature labor.  This has to be treated aggressively.  3 I would suggest that you abstain from heavy activities including sex.  Why sex?  There are two good reasons, 1 the activity can stimulate the cervix causing contractions. 2 semen contains prostaglandins which also cause contraction.  Good luck.

@ nenebran
The normal menstrual cycle is 21-35 days but there is a lot of variation.  It is more important to determine whether you are ovulating with these menses than the interval between them.  As long as you are not bleeding for extended periods of time, I would say you are a normal variant.  Good luck

Dr. Kathy, pls talking about the MC, mine used to be 28 days or more, but recently about four months ago,

it has become irregular after I used Clomid, sometimes 25 sometimes 21 making it difficult to

know my ovulation day. That apart, I will like to know the

effect of this clomid on a baby as I intend to use it again (after 4months) as I have not achieved the aim of

getting pregnant yet. Your response is urgently needed as I'm to start the dosage 2day/tomorrow (clomid)

if I av to use it.

Thanks so much.

I understand the fibroids grow with the same hormones that makes the foetus grow. Is there a risk of both of them growing large enough to affect my internal organs adversely? Am scared Shocked Pls reply

@ Brownbaby

No there isn't any adverse effect of clomid on the baby but it can be associated with menstrual changes.

@titilaelae
Highly unlikely that the fibroids will affect your internal organs.  The organ most at risk are your ureters which contain urine but it is more likely that your fibroids will degenerate because of rapid growth

You need to figure out your menstrual cycle.  A woman's first day of menses is 14 days from the day of ovulation and the egg is only around for 24 hrs.  If you are having sex everyday after ovulation you are just engaging in cardiovascular exercises which will NOT improve your chances of pregnancy.  Start a menstrual calendar so you have an idea when you ovulate.  Also make sure that you are taking a vitamin with at least 0.4mg of folic acid.  Last but not least, take heart.  Most miscarriages are due to genetics and are very common.  Good luck

Thank God that you are here, You are simply God sent. I have two questions

1. My MC is 32 days, When is my ovulation day?, is it the 16th or the 18th day.

2.  Is it possible to get pregnant two days before and two days after ovulation.

Thanks so much.

You ovulation day would be the 18th however you should have sex two days before, day of and day after.  YES you can get pregnant if you have sex two days before because sperm can survive in the female tract.  If you have two days after ovulation, the egg would probably be gone

There are a lot of factors when it comes to fertility.  infertlity is when a couple have been engaged in consistent sexual relations for a full year without conception.  I would suggest you start a menstrual calendar so you can estimate your date of ovulation.  A semen analysis of your husband would also be very useful.
Unfortunately, I am based in NY.  I would have been glad to see you.

My friend, a calendar does not show when you ovulate.  Your body does not recognize our calendar.  You have to create one based on the days of your cycle and then you will be able to figure out your ovulation day

1. she had crawling insects all over her body.worm like movement.

2. she is reducing everyday. she is negative 2 HIV, AA genotype, O+ blood group, xtra test NORMAL SIZE

3. at times stifeness in her legs just can't move the right leg.
