That you Joyce Ucha Kalu 'F' between 1st April 2006 and 30th December 2007, at various area of Ibadan in the Ibadan Magisterial District, in expectation of reward without being a registered medical practitioner did hold out yourself as a medical practitioner and thereby commited an offence punishable under Section 17(1)(a) of the Medical and Dental Practitioners Act, Cap. 221 Laws of Federation of Nigeria 1990.

That you Joyce Ucha Kalu 'F' between 1st of April 2006 and 30th December, 2007 at No. 18  Ayoadesina close, Old Ife road, Ibadan in the Ibadan Magisterial district, with intent to defraud did forge a certificate of registration as a medical practitioner with heading "Nigeria Medical Council Victoria Island Lagos" and thereby committed an offence punishable under Section 467 of the Criminal Code Cap. 38 Vol. 2 Laws of Oyo State of Nigeria 2000

That you Joyce Ucha Kalu 'F' between 1st of April 2006 and 30th December, 2007 at No. 18 Ayoadesina close, Old Ife road, Ibadan in the Ibadan Magisterial district, knowingly and fraudulently utter a forged  certificate of registration as a medical practitioner with heading "Nigeria Medical Council Victoria Island Lagos and thereby committed an offence punishable under Section 468 of the Criminal Code Cap. 38 Vol. 2 Laws of Oyo State of Nigeria
