Custom Application, Urgent Help Needed

please can anybody help me with custom application form. some of the required fields like, ""DATE OF BIRTH, L.G.A, HIGHEST QUALIFICATION DATES AND CODE"" are not opening. i've being trying to fill the form for long now but couldn't because of those fields. i will appreciate it if anyone that has filled the form can be able of an assistant. thanks all.
