Hi doctors in the house, thank you for the good work u've been doing all these while, may God bless u.
my question is- why is it i've a very low appetite for food nd drinks; the point is that even if i want to eat or drink much my stomach wont just accept it, it will be full and its has been like that since i was small. is it that i've small intestines?

Please an Anty of mine has been suffering from partial stroke for about 5 years now, she has been to the hospitals for treatment though one part of her body is affected, but about a year ago, she started experiencing serious pains in that part of her body that was affected. She has taken all types of drugs but the pains keep increasing every day.
Please i really do need your help and response.

secondly, there is a friend of mine that has been suffering from an infection for about 4 yeas now, she coughs, brings out coloured mucus always, with little chest pain, congested throat, short breath once in a while, and the cough is contagious, she has been to verious hospitals for test like chest X-try, sputum test, blood test and others for more than four different occasions, nothing seem to be seen or discovered in the test that has been carried out. Doctor, please, i know u could be of help.

Hi Naijaking1, Balogun nd others,

I want to update u on my situation. I've been trying more to lose weight and it's been working. What I do most is to delay my breakfast till around 11 or 12 then take my second meal around 6 or 7pm and then I wont take anything again for the day, is this a good idea.
One thing I forgot to add to my the problems I listed then was that water comes out from my anus and get's worse when I sweat, it's so annoying and embarassing and I don't know what to do about it.

One last thing is that I read somewhere that sleeping with one's chest facing down puts pressure on the heart and makes it wear out quickly and one thing with me is that if I don't sleep with my chest facin down, I won't sleep on time but since then I've stopped and it has been working for me, the suggested way were sleeping on the back or the right side to reduce the pressure on the heart. Is this really true?

Good to hear that you're loosing weight successfully.

In morbidly obese people, hygiene could be an issue, however, if water is truely coming out of your anus, then you need an evaluation for fistula.

I suspect that excess skin folds, hair follicles produce a lot of sweat to make you think water is coming from your anus.

If the water 'feels, smells like sweat' I would say it's sweat, but if you're passing urine-like material from your anus, then you go for proctosigmoidoscopy(anal and rectal) examination by your md.

Sleeping positions vary, I would stick with which ever position makes you most comfortable.

i read your post. thot i shuld say  somethin. i just graduated in Dec 07, about 2 start my internship.

for your Aunt with the partial stroke of 5 years, i'm  not so sur of what u mean. did she lose the  ability to use one  side of her body completely at the time of s stroke with a slurred speech? has she be doin physiotherapy & speec therapy to regain the use of her limbs and speech.

please explain more about the pain or is it like pins & needlses or burning sensation? is she diabetic and/or hypertensive? is she obese?

all these questions r important and she needs to see a consulatant physician! Smiley

for your friend with a cough of 4 years, i that of TB initially but its a posssibility. does she cough up blood or blood-stained sputum?any significant weight loss, drenching night sweats, loss of appetite?has TB ever been diagnosed and treated in her? u said d cough is contagious TB is extremely CONTAGIOUS! Shocked Shocked

please take her case seriously i;ll advise both of these people to go to LASUTH. Ikeja! they are lot of experienced hands there, ok.

please pray 4 them. God heals, We Care.

she can walk and handle something with the hand and leg affected, but not properly, she has not been doing physiotherapy & speec therapy

according to her, she said the pains are as if a bowl of ice-block is held tight to her bones, and sharp pains. she is not diabetic and/or hypertensive?

No she does not cough up blood, she eats very well, very healthy, no lost of weight. no night sweat at all, in all the test that has been carried out on her TB has never been diagnoised, she was only told its an infection, though i have forgoten what she called it, i think bitter strep or something like that. she was given some anti-biotics injections to take but it didt stop it.

@ controller, I am a doc in pharmacy and i will assist for now. stap aureus is a very serious bacterial infection. i will recommend that if you definitely sure that what you have to start treatment immediately. vancomycin (vancomycin 500mg or 1000mg every 12 hours for 7 to 14 days) is a very good medication for stap aureus (MRSA), and it is the drug of choice. anyother medication is ZYVOX 6OOMG every 12 hours for 7 to 14 days.  The number of days of treatment depends on severity and response to treatment.

@ chii, yes you can conceive during your period, but the percentage of that happening is usually very low. Also, There's no risk to that.

Dr Balogun thanks so much for this wonderful job. please my question is that anytime I'm having sex and i will be having pain at the right side of my abdomen. have been to the hospital my doctor ask me to do scan which i did, later give me augmenting, cefin,etc. but is still paining me. please can this distop me of getting pregnant. I'm afraid, please help.

Soulmatebj. Probably refer your question to my colleague kathyekiti who is doing obs/gyn forum. Would have been best for your doctor to tell you what was found on the scan before starting you on augmentin/cephalosporin. Might be pelvic inflammatory disease!!

Dysfunctional bladder and bowel movements usually characterize spinal cord trauma. Depending on the extent of the injury, symptoms could vary.

In addition to stooling and urinating functions, others such as sexual, and lower extremity motor and sensory functions could also be affected.

In response to your question specifically, diet consisting of high fiber is mostly recommended, because patients tend to be chronically constipated.

There could also be a problem with sphincter muscle control, ie. incontinence to stool, urine, etc.

Congratulations, if you have passed the acute phase of the injury, on the rehablitative phase, you and your family would need a lot of education/understanding of the process.

Physical and occupational therapy would also play a big role.

Depending on your means, there are very good rehablitation programs at Emory, Cleveland Clinic, and of course Mayo Clinic Rochester, MN-World's #1 program.

I studied mostly at Mayo, so my bias may tend towards their program.

Been long since i fired you a question, recently i was back into physical fitness (jogging) and ive been feeling some serious pain in my knee joint which makes me abort the excercise, what do i do or what should i take. I also find it hard to pass shit as when ever i try a hard tiny growth protrudes from my anus blocking the exit path. Thanks

Hi Nairalanders,

I am a medical doctor myself (from www.naijamedico.com) , based in Lagos and wish to give you guys a hand from time to time. So help me God

@big spif, it seems you have constipation with resultant hard feces which can cause you to spend a lot of time in the toilet, as well as experience strange things like "growth protruding" from your anus. Hard feces can cause these. It can also cause painful conditions in you anus if you don't seek medical attention. For all we know, you may have hemorrhoids or other scary things.

My advice? Change your diet. Start with a meals that form little or no stool (soft rice, pap) for about a week. A tablespoon of liquid paraffin three times daily has been known to be helpful. After that, eat meals high in fibres (e.g. vegetables, carrots, cucumbers, meat etc). Reduce the amala or akpu. This should held with the constipation and nip the problem in the bud. Should it persist after 2 weeks, go the the hospital. If at anytime you see blood in your stool, go to the hospital immediately.

As for the knee pain, you haven't given me much to work with e.g. have you sustained any injury on the knee before? How long ago since you last exercised? Did you jump from a height and land on your feet recently? Do you participate in any outdoor sporting event? So many questions. Hope you find these helpful

I appreciate your attention and solving impact on numerous issues here. I want to  know he problem with my noisy stomach. I always feel and hear this noisy sound of worm in me mostly during the day and almost any time my attention is drawn to it. Could it be some other worms, other than usual ring worm, because I have taking meds for this and the story is still the same, or could it be pile, not sure but I call it jedi-jedi because I have used serial meds for that too, even herbs that considered strong for it.
  Please advice me if this is some different case to what i thought or a different word from normal ring worm, and relatively need different medical measure.

May God bless you for the help you've given Nairalanders.

I developed a kind of allergy sometime last year.
When i'm cold, slight inflammation erupt from my skin and it itches like mad.
It emerges like rashes on a line, sometimes, a line, sometimes the mark of a cane.
The more i scratch it, the more the swelling extends to other areas of my skin.
The swell  looks like mosquito bite.
It is worse at night.
I was told by a trado-herbalist that it is a symptom of impending fatness.
Please help!

I got slapped real hard on my left cheek about two months ago and since then, i have been having regular, persistent and throbbing pains in my left ear. Please,kindly recommend any medication that will stop the ache. Thank you.

Lol. What has loose vagina AFTER CHILDBIRTH got to do with slaps?

You should ask questions before jumping to wrong conclusions. Read my previous posts on other thread-i was attacked by armed robbers. I just didnt think the slaps will have any after effect until i began having serious ear aches.

1.i want to know the physical symptoms if any

Unfortunately, low sperm count hardly presents with any physical symptoms. It is usually discovered accidentally during a semen analysis when a couple complains of inability to conceive after over 1 year of frequent, regular unprotected sexual intercourse.
Different treatment modalities exist depending on the exact cause. In developed countries, assisted reproduction is used to solve this problem. This technology is available in nigeria but it is rather expensive. It involves taking the man's sperm cells from his semen and directly introducing it to the woman's eggs either inside out outside her body.
Many factors contribute to low sperm count but some common ones in this environment include wearing tight underpants/trousers, untreated testicular/scrotal conditions whether acute or not e.g. torsion of testes, having mumps in childhood, untreated STIs, etc. Infact, a medical expert from UCH Ibadan just alluded to "Okada" riding as a cause of low sperm count!

Someone close to me had her 1st baby through c-section.She got pregnant again after 1 year and her doctors says she shold have waited for 18   ismonths.that she is likely going to undergo another c/s.Please is 1 year too early for another pregnancy?

@ChiefS, her having another C-section is dependent on the reason for which the previous one was done. The 18 months is to allow her more time to recover and be fully prepared for the stresses of another pregnancy
