The problem could be liver cirrhosis. Does he smoke? Drink? Such things cause bleeding like that. Is the blood a dark-rediish colour?

I would suggest that your brother should go for a comprehensive medical check up , let it be well established that he is suffering from a particular disease, you should not just sit down in a corner and begin to speculate. If it is a liver problem let him go for medical tests to prove that he has it and is it chirrhosis, or which kind of hepatic disease it is, let this be confirmed first.
After which you now seek for treatment. For more enlightenment on this, you can call this number; 08033082395.

Like pappyjebby suggested, he should be taken to a specialist hospital for a comprehensive checkup, LUTH for instance or any University Teaching Hospital and if he can't be treated, he'll be referred to a better hospital, most likely outside the country. that is of course depending on the disease and care needed.

Pray for him too.

Good luck ok.
