Please friend where can i get a yellow fever card in lagos,nigeria. And is it must i that i must go with it to south africa ?

Go to dept of public health at the international airport.Once you enter the departure lounge,ask for where post office is,its opposite post office.
Go with your international passport and there they will give you the yellow fever injection and your yellow card for a small token,N1k.
I advise you obtain your yellow fever card at the airport rather than at the local govt. health centers as their vaccine batch numbers may not tally with the ones they inoculate you with at the airport.

My fellow Nigerian,I said go to the public health dept at the airport for both your yellow fever card and injection.Not at information desk or airline counter.shey you don grab?

A passport or travel document valid for no less than 30 days after the expiry of your intended visit
Your passport must have at least one unused page for entry / departure endorsements
A completed Form BI-84 (application for a visa)
Payment of the prescribed fee
A yellow-fever vaccination certificate (if required)
Statement and/or documentation confirming the purpose and duration of your visit
Two colour passport photographs
A return or onward ticket if you are travelling by air
If you have children (minors) travelling with you or joining in South Africa, you will need to provide the following additional documents

Proof of guardianship or custody or
Consent from the guardian in the case of an unaccompanied minor
Proof of financial means to pay for your living expenses while in South Africa in the form of:

Bank statements for 6 months (Please note that all applicants must only submit an original bank statement with an official stamp and a signature by the bank official. Further all Bank statements that read as BANK PHB will not be accepted by the South African High Commission. All other Bank statements including Keystone Bank would be accepted.)
Salary advances
Undertakings by the host(s) in South Africa
Bursary
Medical cover or
Cash available (including credit cards and travellers’ cheques)

Requirements for entering South Africa

You will need the following if you wish to visit South Africa:

A valid and acceptable passport or travel document for your intended stay
At least one blank page in your passport for endorsements
A valid visa, if required
Sufficient funds to pay for your day-to-day expenses during your stay
A return or onward ticket
Yellow fever certificates if your journey starts or entails passing through the yellow fever belt of Africa or South America

The yellow fever card is #1000 only. They don't work on weekends

However, the location for obtaining the vaccination and issuing the Yellow Fever card has changed. The location is Ministry of Health, Port Health Office, beside the Toll Gate. If you are coming from Mafoluku Oshodi axis, turn right into the toll gate. The building is immediately after the toll gate. You should see the sign post.

The procedure is also very simple and straightforward. You will need a photocopy of your Passport Biodata page and N1000. You have to go in person as proxy processing is not permitted.
1. Enter your name, Passport Number and Telephone Number in the Register provided.
2. Submit the photocopy of your Passport biodata page and N1000 to the receiving Nurses.
3. Wait on the queue for inoculation.
4. Get inoculated.
5. Collect your Yellow Card.

As expected, there are touts around the premises waiting for gullible people to fleece. Don't fall victim. The process is simple and easy. It takes less than 5 minutes.

Find your way to IKEJA POST-OFFICE,beside there is a motor park where buses going to airport are situated....tell the driver you are going to PORT-HEALTH




MY MAIN QUESTION IS....
I was to submit YELLOW CARD with other of my docs to an embassy around February but I later changed my mind due to timing with my work and other logistics.
Now I'm ready to do the submission, knowing fully well that travel date is stated on he YELLOW CARD.

*Fellow....Please can I still use the previous YELLOW CARD or il need to do another.

So you mean I cant travel before 10day elapses.

I intend doing on tuesday and making a quick trip on Wednesday. Is that out-rightly impossible?

You can explain your situation to the personel
Incharge of payment and registration that you need to back date the date of issuance as the case may be she will request for an additional token, but she will do it for you, 500/1000# will do the magic

Beware of touts sha o, they are many in that place, make sure you enter into the main lounge, register your data page, q and take your vaccine, pay an sit patiently till your name is called for pick up of ur yellow card, it's only for 1k
