Open your SBM and go to Financial Plan page and copy the amount in line with Beneficiary Contribution and put it in the field provided on the web page, do the same to IDA column too.
Only 1st page of your Form C will be required, this is the part where you already provided or answer the check list.

GUYS please log in to your acoount and see if you have similar issue like mine. My account is locked ''We can not verify your GUYS training Certificate through our data base,your account will be locked until we can verify your certificate'' . Am seriously weak.
