Barotrauma refers to injury sustained from failure to equalize the pressure of an air-containing space with that of the surrounding environment. The most common examples of barotrauma occur in air travel and scuba diving. Although the degree of pressure changes are much more dramatic during scuba diving, barotraumatic injury is possible during air travel.

Barotrauma can affect several different areas of the body, including the ear, face and lungs. Here we will focus on barotrauma as it relates to the ear.

What are the Symptoms of Barotrauma?

Symptoms of barotrauma include “clogging” of the ear, ear pain, hearing loss, dizziness, ringing of the ear (tinnitus), and hemorrhage from the ear.

Dizziness (or vertigo) may also occur during diving from a phenomenon known as alternobaric vertigo. It is caused by a difference in pressure between the two middle ear spaces, which stimulates the vestibular (balance) end organs asymmetrically, thus resulting in vertigo. The alternobaric response can also be elicited by forcefully equalizing the middle ear pressure with the Politzer maneuver, which can cause an unequal inflation of the middle ear space.

Inner Ear Decompression Sickness

Inner ear decompression sickness (IEDCS) is an injury that closely resembles inner ear barotrauma; however, the treatment is different. This injury is more common among commercial and military divers who breathe a compressed mixture of helium and oxygen. Symptoms include hearing loss, ringing of the ears, and/or dizziness during ascent or shortly thereafter.

IEDCS most often occurs during decompression (ascent), or shortly after surfacing from a dive. In contrast, barotrauma most often occurs during compression (descent) or after a short, shallow dive. Patients with IEDCS should be rapidly transported to a hyperbaric chamber for recompression. A significant correlation exists between early recompression and recovery.

Rheumatoid arthritis (RA) is a disease that causes pain, stiffness, swelling, and loss of movement in your joints. It happens most often in the wrists, knuckles, knees, and feet. It can also affect other parts of your body.

RA is a lifelong problem that usually starts in early adulthood or middle age. You may have just one attack, but more often the symptoms come and go. Repeated attacks can lead to permanent joint damage. You can relieve symptoms and prevent or slow down joint damage by following your healthcare provider’s treatment plan and taking good care of yourself.

WHAT IS THE CAUSE?

Rheumatoid arthritis is an autoimmune disease. This means that your body's defenses against infection attack your body's own tissue. When you have rheumatoid arthritis, the attack is mostly against the tissues that line the joints. The tissues get inflamed, causing pain, swelling, and stiffness. The inflammation also damages bone and cartilage (the cushioning in joints) and can change the shape of your joints.

Things that may cause or contribute to rheumatoid arthritis are:

Genes you have inherited. Genes are inside each cell of your body and are passed from parents to children. They contain the information that tells your body how to develop and work.
Infection, such as infection with the Epstein-Barr virus or rubella (measles) virus
Levels of female hormones. RA is more common in women. The symptoms tend to get better with the higher hormone levels that happen during pregnancy. Then the symptoms often come back after delivery, when hormone levels return to normal.
Smoking, which may trigger an auto-immune reaction
Long-term exposure to chemicals, like silica or asbestos.

I am here to speak with you on Prostate. The topic is misleading. Is prostate strictly for men? Yes, ONLY men have prostate and ONLY men over 40years but the healthcare enlightenment is for everyone. There is no woman who does not know a man 40 years and above – father, uncle, brother, son, friend, neighbor, colleague …

Essentially what I will be doing today is health promotion. Responsible health promotion must provide three things:

1. Information
2. Reassurance
3. A plan of action.

Let me start with a background on prostate health.
Everyone has a pair of kidneys. The job of the kidney is to remove waste. It is the LAWMA (waste management company) of your body. Everyday your blood passes through the kidney several times to be filtered. As the blood is filtered, urine is formed and stored in a temporary storage tank called the urinary bladder.

This is commonly called tailbone pain or coccyx pain, is a fairly rare and relatively poorly understood condition that can cause persistent pain at the very bottom of the spine. This part of the spine is the coccyx, or tailbone. Coccydynia is felt as a localized pain and will generally feel worse when sitting or with any activity that puts pressure on the bottom area of the spine.

The condition is much more common in women than men. It is usually caused by local trauma (e.g. a fall) or giving birth. On rare occasions, an infection or tumor can also cause pain in the coccyx.(Here there is a video about coccydynia causes and medical treatment

Coccydynia symptoms may consist of one or all of the following:

Pain that is markedly worse when sitting
Local pain in the tailbone area that is worse when touched or when any pressure is placed on it
Pain that is worse when moving from a sitting to standing position
Pain that is worse with constipation and feels better after a bowel movement.
A combination of treatments to reduce the pain and activity modification to keep pressure off the tailbone usually suffices to control or alleviate the pain. In very rare cases, surgery to remove the coccyx may be recommended, but the surgery (a coccygectomy) will typically only be considered if the pain is severe and at least several months of non-surgical treatment and activity modification has not been effective in relieving the pain.

Treatments for coccydynia are usually noninvasive and local. The first line of treatment typically includes:

Non-steroidal anti-inflammatory drugs (NSAIDs). Common NSAIDs, such as ibuprofen, naproxen and COX-2 inhibitors, help reduce the inflammation around the coccyx that is usually a cause of the pain.
Applying ice or a cold pack to the area several times a day for the first few days (2 weeks ) after the pain starts.
Applying heat or a hot pack to the area after the first few days ( when redness,warmth or swelling is absent).
Avoiding sitting for prolonged periods, or placing any pressure on the area, as much as possible.
A custom pillow to help take pressure off the coccyx when sitting. Some find a donut-shaped pillow works well, and for others it is not the right shape and still puts pressure on the coccyx. Many prefer a foam pillow that is more of a U-shape or V-shape (with the back open so nothing touches the coccyx). Any type of pillow or sitting arrangement that keeps pressure off the coccyx is ideal.
If the tailbone pain is caused or increased with bowel movements or constipation, then stool softeners and increased fiber and water intake is recommended.
Stretching-Gently stretching the ligaments attached to the coccyx can be helpful. A physical therapist or trained healthcare practitioner can provide instruction on the appropriate stretches. (Select from sacroiliac joint treatment, focus on extension exercises)
Ultrasound- Physical therapy with ultrasound can also be helpful for pain relief.
