I have always said it... if you are a frequent viewer of the plane crash show on discovery channel you will agree that 90% of plane crashes are as a result of human error. Very sad indeed how often as humans do we ignore warning bells?

If there are no Plane crashes, there won't be Black box analysis. Why walk through Refurbished Airports to board rickety, unsafe Planes??

There's a limit to what the minister and NCAA can do. The pilots, airline engineers,etc also have some responsibility. The minister or NCAA cannot accompany every aircraft to press the right buttons and take decisions for pilots. Assuming a pilot decides to commit suicide or ignores very important warnings, how is the minister or NCAA supposed to know or remedy the situation? That is why they call it SHARED RESPONSIBILITY. Everyone has his responsibility towards ensuring safety. The minister cannot take critical decisions for a qualified pilot and vice versa. Dont forget the mentality of "I can manage the situation" of the average Nigerian.

For a pilot to be called a captain,he must av accumulated 6,000 hrs of flight.....maybe 'i too know' syndrome dey worry him,or he feel he can manage d flight,afterall he shld be in akure in 25mins....i believ its d captain fault,he is d most superior on board,n has d final say then,his wealth of experience deceivd him..God knws best

Na so Nigeria bad! to the extent of 'doctoring' a blackbox! I doff my hat.
I do remember a naija pilot who ignored a bad weather report and put the aircraft on the runway, took off and eventually had a mishap. Yeah, some pilots do risk lives by ignoring warnings.

So accroding to this report, it was due to mechanical failure..

The pilot receive a warning of errors on his flap after he had hit 8 knots during the take-off roll.. Questions arising:

1. Why did the pilot not abort the take-off?
2. Was it possible for the pilot to safely abort take-off (maybe he didn't have enough runway left)
3. Did the pilot get a warning on the low power being produced by one of his engines?
4. Was the faulty engines condition a pre-existing one or did it just pop up (remember that the plane was on its second sortie of the day).
5. Was all the preliminary flight inspection prior to take-off done (the black box should have this record)..


From this report, the NCAA and the Aviation regulatory agencies are not looking guilty..
