Required information for GIFSHIP

1. Surname
2. First name
3. Middle name
4. Date of birth (date/month/year)
5. Accredited HCP of choice (the hospitals does not need to be the same or in the same state)
6. State of origin
7. LGA of origin
8. State of Residence
9. LGA of residence
10. Phone number
11. Email address
12. Passport photograph of all that want to be registered

GIFSHIP is an annual renewable program, this means that after first registration, renewal should be done every year.

All your details will remain the same for this one year so think wisely before pening down things especially like name, age and the hospital you choose to use.

* consider your location and the service you require from a facility before choosing your HCP. Your residence should not be abeokuta, ogun state and you are choosing a hospital somewhere in the island of Lagos state. Some people actually plan to move before the registration waiting period is over and decide to still choose HCPs closer to where they want to move away from. Some people don't make proper checks on the HCP they choose and later tell you they didn't no the hospital was far or some other reason which they would have investigated before choosing the HCP.

* If you are using a certain name in a hospital then during your registeration with NHIS register with the same name and if you are already registered with NHIS then use the same name to register in your hospital of choice.

* Date of birth and sex should also be the same with your registeration in your HCP of choice.


* After filling your registeration form, go through the form once or twice again to be sure you put in the right details for everyone.

* Using clear capital letters to write your details down, this will help your details to be clear for the operator that will register you.

I'm a federal worker under nhis and currently pregnant and about to deliver. Due to the fact that I will be relocating I applied for a change of hospital from fmc in ondo state to fmc in Ogun state. I reside in Ondo state but will be moving over to Ogun state.
I went to nhis office did all I was asked to do and the officer in charge told me he has changed it that I should check back by 1st week of July that if is out by then then I will be knocked off from fmc ondo to fmc ogun.
My regrets now is I wish to give birth in Ondo b4 moving to Ogun state. Supposing the change and transfer comes out successfully by 1st week of July can I still remain in Ondo and give birth under the nhis without moving to the fmc Ogun I was moved to?

You will be treated as a transit patient/emergency

Once your name is out on the NHIS register you can start anti natel in your hospital of choice and also delivery will be covered BUT no surgery including CS till after 6months that your name comes out on the register

Renewal is when there is no change in the previous registration. So now he is going to register a fresh one as 2people are no longer interested, remember whether it's just him or with one or 2 other people, price remains #45000. If he wishes to do the renewal anyway it means the other 2 peoples registration will be done along with his. Renewal takes at least a month while registering afresh is at least 90days

But the problem is; they were told not to change hospital.
Can they use it in another hospital for long term admission ( minimum 30 days)?

GIFSHIP renewal is possible when same people, same names, same HCP, same HMO want to continue the next year registration.

Once any of the detail above changes the only a fresh registration can be done, it is no more a renewal.

If it formal sector then you have to stay in you current hospital for 6months

The child should either be in a higher institution or serving as a corper which are covered under NHIS or you can register the child for GIFSHIP
