It varies from state to state. In Gombe State, we only Went with our original credentials and duplicates and a passport photograph.

I have done my verification nothing of such, just my Nysc, Waec and degree cert I presented, I was even ask to choose hospital that I prefer to work. Am in port hacourt.

go to ministry of health at ph city local govt at Lagos ( not Lagos state) beside ph mall, go with your Nysc cert, Waec original, degree and they wil verify you immediately and you have the option to choose a hospital close to you, the women there are friendly
