Then you should have continued that way instead of changing it again. About the name issue, i don't think it is much of a problem though.

please i applied using etisalat number but used Mtn as an alternative number... and were using 4point scale.. so for Cgpa I wrote it as 3.95/4.00.. could that be a problem?

Age won't have any effect if u finally win the scholarship. Most of the credentials they will ask for are ur school ones like School ID card or Admission letter, Ur Transcript and ur Course form
