Nutrition assessment form is a tool used in OVC programming. Learn now how to fill nutrition assessment form in OVC programming.
Nutrition is one domain on the service form that is very important in OVC programs. However, the nutrition assessment form is used both in LOPIN 1 and LOPIN 2 project of the USAID/UNAID sponsored project.
Some of the services and plan interventions include:
Nutrition education and counseling
Food & nutrition supplements
Vitamin A, Zinc iron & supplements
Nutrition assessment, counseling and support
Growth monitoring: Height (cm) and weight (kg)
Nutrition referral – malnutrition.
For a clearer understanding of each of the services under the nutrition domain of the service form, check below what each of them is all about.

Nutrition education and counseling:
Nutrition education and counseling basically have to with the sensation you have either given to the caregiver/household head or orphan and vulnerable children (OVC). It includes counseling the vulnerable household on how to prepare meals that contain adequate diet. It also includes sensitizing them about how to combine the handiest, affordable and available food items that could nourish their body with proper diet per time. Remember they are vulnerable so you don’t advise them to buy expensive things which they would not be able to afford. Always build on their strength and not their weakness.

Food & nutrition supplements:
This has to do with food supplements that serve as what could be called boosters or energizers. These go a long way to fast track on rebuilding a vulnerable person from a malnourished state to being healthy. It is important to always come back to your service forms/tools for each of the vulnerable household members of a particular household to document and tick appropriately after you have provided food and nutrition supplements.

Vitamin A, Zinc iron & supplements:
This is just about the same thing with the one above. The difference is just the fact that this one basically has to do with building the blood levels of a vulnerable caregiver/OVC to restore health back. It also has to with restoring energy too.

Nutrition assessment counseling and support:

After providing counseling support to a caregiver/OVC under your care, it is always advisable to conduct frequent checks for assessment on the nutrition state of the household under your care. Check for improvements on the person you are dealing with. This simple observation is a service you have provided so you can go back to document it on the service form.

Growth monitoring: Height (cm) and weight (kg):
Growth monitoring is done using the BMI. BMI which stand for Body Mass Index, is a tool that is used to calculate and estimate the body size of a person with the help of a mathematical formula. The BMI instruments for measurements are scale and measuring tapes.
The instrument for determining the health of a person via the weight and height squared was first discovered and adopted around the 19th century. This is with reference to the Archives of Disease in Children, an article published in 2006 about BMI.
However, the formula for calculating weight and height is- BMI= kg/m², where kg is the person’s weight in kilograms, and m² is their height in meters squared. BMI commonly applies to adults from 15years of age.
