How Nigeria’s 36 states fare in HIV prevalence

Two South-south states are among the three states with the highest HIV prevalence rate in Nigeria.

Akwa Ibom tops the prevalence rate chart with about 5.5 per cent of its residents living with the virus. It is followed by Benue which has a prevalence rate of 5.3 per cent while Rivers is third with a prevalence rate of 3.8 per cent.

The details were contained in the Nigerian HIV/AIDs Indicator and Impact Survey (NAIIS) whose result was released last week in Abuja.

The result shows that about one per cent of Nigerians (1.9 million people) are currently living with HIV/AIDs.

This figure, according to the Minister of Health, Isaac Adewole, moves Nigeria from second to the fourth position among countries battling the HIV epidemic.

With an earlier estimate of about 3.2 million people living with HIV, Nigeria was second after South Africa which ranks first with about 7.1 million people living with HIV.

Announcing the NAIIS result in Abuja on Thursday, the Director General of the National Agency for the Control of AIDs (NACA), Sani Aliyu, said the percentage of People Living with HIV among adults is 1.4 per cent (1.9 per cent among females and 0.9 per cent among males.)

The National AIDS survey was launched in June 2018 to determine the true distribution of HIV and AIDS, Hepatitis B and Hepatitis C in the country.

The survey reached around 220,000 people in about 100,000 households and was conducted between July and December 2018.

According to the survey, the South-south zone of the country has the highest HIV prevalence, at 3.1 per cent among adults.

HIV prevalence is also high in the North-central zone with 2.1 per cent and in the South-east zone, 1.9 per cent.

The three regions with lower prevalence rate are the South-west – 1.2 per cent, North-east – 1.1 per cent, and the North-west zone – 0.6 per cent.

The Human Immunodeficiency Virus (HIV) is a virus that attacks cells in the immune system, which serves as the body’s natural defence against illnesses.

Though the popular way of contracting this virus is through vaginal or anal sex, there are other ways. These include sharing of needles or syringes, piercings, tattoos, blood transfusions, semen (cum), or vaginal fluids into open cuts or sores on the body. HIV currently has no cure but can be prevented and managed with proper medication.

1. Stop crying. Having high prevalence does not mean you are evil. My state is among the top three...and I am not crying.

2.Since 2017, there has been an increase in adherence, counselling, and aggressive treatment of positive patients.

3.The result you are posting is estimates. In 2018, there was a survey which took some months...and the results is what was posted by the op.

4.You can keep on crying...or you can tell your state governor to put pressure on the Ministry of Health and the Minsitry of Educaiton to educate, educate, EDUCATE....disasters can always be reversed.

Note...the survey was carried out to update records. It is more detailed than what you are posting.

The figures you are quoting up and down are from 2007-10. A lot has changed since then...and it is limited to adolescents.

HIV and AIDS in Nigeria

Six states in Nigeria account for 41% of people living with HIV, including Kaduna, Akwa Ibom, Benue, Lagos, Oyo, and Kano.6 HIV prevalence is highest in Nigeria’s southern states (known as the South South Zone), and stands at 5.5%. It is lowest in the southeast (the South East Zone) where there is a prevalence of 1.8%. There are higher rates of HIV in rural areas (4%) than in urban ones (3%).7

Approximately 150,000 people died from AIDS-related illnesses in Nigeria in 2017.8 Since 2005, the reduction in the number of annual AIDS-related deaths has been minimal, indicative of the fact that only 33% of those with a positive diagnosis in Nigeria are accessing antiretroviral treatment (ART)

Again you are quoting pre 2018 estimates

The test the OP posted was done last year...from June 2018, and involved 220000 persons across hundreds of hosueholds.

When you have a problem you don't cry, you do something about it.

And speaking as a medic, what we want is HIV prevalence of 0%. That's a success for us. So even 1.3 % males us miserable.

PERFECT REMEDY FOR HIV POSITIVE PATIENTS

God Bless You Dear Readers.

What Is HIV?

It is a virus that causes damage to the immune system. But as you know, the immune system is supposed to protect us from infections.

And when you fail to treat HIV, it affects as well as kills CD4 cells, an immune cell called T cells. And as this continues, the body continues to get all types of infections and cancers.

For more on this please follow this link

cheesy cheesy cheesy cheesy

Here is something about statistical surveys

1.The sample size is determined.

2.It is from the sample size that the data is gotten

Since they cannot go to EVERY house in Nigeria...they took a sample size containing some of the households in Nigeria, and extrapolated the results from there.

That's how surveys are done.

To put it simply, say I want to do a survey of the weight of 10 YEAR OLD children in schools in Benue state.

1.I take a sample of some of the Local governments area....ie I decide i will study five out of the 25 lga

2.I take a sample of villages in each LGA...in one LGA , I will study four villages.

3.In each village I will reach 30 households.

the data obtained is what I would use to say...The average weight of 10 year old children in benue is 45KG.

If they went to every house in Nigeria...it would be impossible from a logistic point of view.

https://healththink.org/hiv-aids-in-adolescents-and-young-people-in-nigeria/

A chart showing UNAIDS 2017 estimates of new HIV/AIDS infections among adolescents

An undeniable observation is the fact that estimates of new HIV/AIDS infections among adolescents are more prominent in Sub-Saharan Africa – the region most affected by HIV. AIDS is the number one killer of adolescents and young adults in Africa 2. However, recent data show that only 13 per cent of adolescent girls and 9 percent of adolescent boys aged 15-19 have been tested and received their results for HIV in the past 12 months 3. Furthermore, over the past decade AIDS-related deaths have increased disproportionately in adolescents while decreasing among other age groups 5.

Nigeria follows South Africa as having the second largest HIV burden in the world with a prevalence28. This prevalence rate differs by region. In the South-South region, the prevalence stands at 5.5% which is the highest compared to other regions, while The South-East region has the lowest with a prevalence of 1.8%27. Adolescents and young people make up 31% of the entire population of Nigeria. Adolescents between the ages of 10-14 make up the largest proportion compared to other age groups 27. Data from the Nigeria AIDS Control Agency (NACA) put the prevalence at 4.2% for young people aged 15 to 2411. Recent estimates put the number of adolescents- aged 10-19 years- living with AIDS to be 160,000. National data also suggests that 40 percent of all reported new cases of HIV occur in young persons aged 15 to 24 which is the highest when compared to other age groups 7.

Source: Adapted from NACA- National HIV strategy for adolescents and young people27

Table 1 shows the HIV prevalence by regions in Nigeria. It is apparent from the table above that the prevalence of HIV/AIDS among adolescents and young people differ remarkably by regions.

Awofala and Ogundele29 suggests that these differences in prevalence rates may be because of factors such as levels of education, religion, socioeconomic status and cultural diversity. The prevalence of HIV also varies markedly by gender, as young women are more infected with HIV than their male counterparts. This is especially true in the South-South region where young females aged 20-24 years have a prevalence as high as 7.5% compared to their male counterpart with a prevalence of 3.5%.

Modes of Transmission

The WHO30 categorised the mode of transmission of HIV in adolescents and young people in two ways- vertical (mother-to-child) transmission or horizontal/sexual transmission.

Vertical Transmission

According to an estimate, globally, about 90% of HIV positive adolescents acquire the infection perinatally31. In Nigeria, vertical transmission of HIV is the second most common mode of transmission of HIV after sexual intercourse27. Despite the contribution of perinatal infection to the incidence of HIV infection in adolescents and young people, it is seldom studied by the scholarly community and its impact remains unknown.

\horizontal/Sexual Transmission

Sexual transmission of HIV is the second most common route of HIV transmission in young people. It accounts for about 50% of the 35.3 million under 25 young persons living with HIV32.

Early sex initiation is a factor which increases HIV vulnerability in adolescents and young people. Recent estimate suggests that 15% of young persons start having sex before the age of 15 years33. Another important driver of early sexual debut is child marriage. Girls as young as 15 years are given away in marriage and give birth to their first child before 18 in some parts of Nigeria35. Furthermore, the very low knowledge of HIV status among young people in Nigeria may be a huge contributing factor32

Condom Use and Contraceptives

The prevalence of condom use among adolescents and young people in Nigeria is low36. Data from the National HIV and AIDS and Reproductive Health Survey (2013)37 show that 56.4% of adolescent boys and 39.6% of adolescent girls had unprotected sex with their partners. What is striking about these figures is that more adolescents were engaged in this behaviour than other age groups in the study.

Young people are also part of high risk populations such as sex workers (SW), men who have sex with men (MSM), people who inject drugs (IDU) and people who are transgender. In Nigeria, studies examining the prevalence of HIV/AIDS in adolescents and young people in these high risk populations are few.

HIV Awareness and Risk Perception

Evidence suggests that Nigerian youths have a high level of awareness of HIV/AIDS, especially with regards with prevention and care12-16. Studies showing a low level of awareness with regards to HIV/AIDS is uncommon17. These existing studies however fail to resolve the difference between awareness of HIV/AIDS and a comprehensive knowledge of HIV/AIDS. It is important to differentiate between these two because of the misconception about HIV/AIDS that is common. A 2015 UNICEF report23 shows that only 34% of adolescent males in Nigeria have comprehensive knowledge of HIV, while only 24% of adolescent females have the necessary and accurate information on HIV. Statistics show an awareness-knowledge parity index of 0.39 in South East, 0.55 in the North-West, 0.91 in the South-West, 0.67 in the South-South and 1.49 in the North-Central.20

Most HIV/AIDS school programs rely on education and behaviour change programs. Even though these programs help provide awareness and comprehensive knowledge on HIV/AIDS, there is a growing body of evidence that suggest they are weak in controlling risky sexual behaviours 18,19,20,21,22. These can be seen in studies that have shown increase in the sexual activity of adolescents,24-26 and decrease in the utilization of condoms and other contraceptives 21-23. Some studies have shown that the disparity between knowledge and behaviour may be responsible for the lingering spread of the disease in Nigeria38-41. This view is supported by Smith who argued that “the disconnect between young people’s knowledge about HIV/AIDS and what they do results because risks is constructed on the foundation of ethics and morality which is usually interlaced with morality shared on the community level and subjective assessment of ethics of personal behaviour”42(p. 345)

Addressing the HIV epidemic in adolescent and young people

A new strategy to end the AIDS epidemic was launched in 2015. This new global strategy focuses on addressing the epidemic among adolescents and young people. It is titled ALL IN! to End Adolescent AIDS and aims to accelerate the global AIDS effort and end the AIDS epidemic among adolescents and young people by 2030. The key aspects of the ALL IN! aims to reduce HIV in adolescents and young people in 2020 by a 75% reduction in new HIV infection among adolescents; 65% reduction in AIDS related deaths among adolescents and ending stigma and discrimination. To achieve this, appropriate proven programmes were introduced for each defined agenda in target countries. The main features of the programmes are:

HIV testing, treatment and care in adolescents and young people
Combination HIV prevention in adolescents and young people

In Nigeria, a national HIV strategy for adolescents and young people was developed to stem the tide of HIV infection in adolescents and young people. Factors that contributed to the lingering HIV crisis in adolescents and young people were identified. They include negative attitudes of providers towards adolescents and young peoples’ sexual activities, confidentiality and bias, access to youth-friendly services with specially trained sensitized health care workers, socio-cultural norms about sex and poor health-seeking behaviour among adolescents and young people27. Adolescent and young people HIV/AIDS program in Nigeria has a goal to reduce new HIV infections in adolescents and young people by 2020 by focusing on the following areas27:

HIV counselling and testing
Prevention of mother-to-child transmission (PMTCT)
Prevention of sexual transmission of HIV and other STIs
Treatment, care and support

Conclusion

The World AIDS day which is held on 1 December every year presents a unique opportunity and a challenge for everyone to unite in the fight against AIDS. Young people are the key and need to be engaged in order the stem the tide of the HIV/AIDS epidemic43. In Nigeria, although a national strategic plan to address HIV/AIDS in adolescents and young people exists, young people are rarely engaged, and some key populations are marginalised in these strategic plans.

While most people in the South East and South South go to hospitals to get tested for HIV/AIDS,people from other geopolitical regions normally do not especially in the North East and North west
