It is done within the family court and the social welfare offices. Firstly, you file an application and submit the following documents to the social welfare office:
1. Medical certificates of fitness
2. Indigene certificate
3. Birth certificate
4. Evidence of means of livelihood
5. Pay slip or bank account for the last 3 months
6. Two passport photographs
7. Letter from 3 referees
8. An affidavit
9. Marriage certificate
10. Consent letter from the biological parents of the child with their passport photo
The application fee is N700. After filing, the Social Welfare officers investigate the circumstances surrounding the application and give their report to court. The court will then give its decision.
