I don't see the effect on him sexually as people claim. We stay days, weeks, sometimes months without sex. And when we do, it's within mins like normal people do.

Tramadol addiction is quite hard to quit especially when one is dependent on it.
I personally experienced it, tried to go cold turkey at different points but couldn't, until I worked out a plan for myself and I've been clean for 4 years now.

It wasn't a walk in the park but with dedication and discipline, it can be done.

I can help you work out a plan though but would still need more specific details on his case. Give or take, in few months, he can be free.

The only way to help him is to tell him the consequences of his addiction and report it to necessary authorities.
(Parents, church leaders, even the government )
From what you've typed , you're already on the road to becoming his co addict.

The person that unconsciously motivates him to use tramadol.

Stop trying to control his life,
Have no reaction to his addictions, if he takes it, he's an adult, he knows what he's doing, if he doesn't take it, he also knows what he's doing.
Quit trying to save people, you can't.

The more you try to exercise control over him, the more he continues the addiction to keep you dedicated to him
