Sorry, OP. Seeing any of my pets like this would definitely break me. Have you taken him to the hospital for diagnosis/tests? Any prescriptions yet? Also ensure whatever it is is not transmissible. You should take extra care of him after this knowing how prone he is to diseases. Please, don't kill it like people are suggesting (it's funny how some people would be in their houses and be advising you to kill a companion without caring to know what he means to you). However, if you ever decide to call that route, take him to a vet for mercy killing (euthanasia).

The vet that came gave him his normal shots and applied antibiotic spray. Also gave him some anti biotic shot.

Heard powdered snail shell is really good in healing this sort of gash...

Great!

That's just severe mange from mites. Anti-biotics and drugs for treating mange would do just fine. I had a dog whose case was even worse than yours, but he survived. Just make sure he doesn't keep itching that area. He'll be fine.

It will survive, just calm down and put your mind at rest, I'll tell you what to do.
1) Firstly ensure the affected area is clean and disnfected by using a clean cotton wool and mild antiseptic
2) Apply Gentian Violet (GV) to the affected area, ensure the area is always covered with GV
3) Please do not attempt to cover the wound with bandage or plaster else, it will "cook the wound" (provide foci for bacterial proliferation)
4) Administer a prophylactic broad spectrum antibiotics, this can be gotten in a Vet pharmacy but if you have a clinic nearby it's better it's via IV (but since your Vet had administered it already, just continue with the treatment)
5) Treat ectoparasitic infection like mange etc, I believe your vet might have done that already with Ivermectin you said he administered
6) For now avoid any bath like cypermethrin or armitraz bath, they may irritate the wound or optionally cover the wound during the bath
7) Administer multivitamins especially Vitamin C and Bcomplex
Tender loving care and very good/nutritious food especially with fruits and vegetables
9) Be patient and put your mind at rest.

The dog will survive it. You need restrict his movement and head so the wound can heal fast. Thanks for trying your best to care for him. They know you're trying to save them as they have feeling. Make sure you get the best assistance from a good Vet as most just parade around as Vet
