Is it okay to simply give the reference numbers of these denials or I have to give details of the reason why?

Yes you can either include IRCC or leave it blank. By default, ICES grants CIC electronic access to your evaluation report wether you so permit or not. Mind you it's not electronic access to your personal info but your ECA Report ONLY. I hope this helps.

From the bolded, I think it's safe to say that you should explain why

It depends on whether you did coursework during your PhD program. In my case I did coursework in Unilag and my PhD transcript was sent for verification. If transcript isn't available, your institution may send a 'letter of award'.

one question to the FAM. Hubby spent 4 days in Italy, in 2014 on a tourist visa. do we need to get a PCC for it?
read somewhere that, PCC is only required if one spends 6months and above in a particular country.

I suppose you also clicked on Courier delivery within Canada which explains the extra 26cad added to you supposed 275cad bill.
Review your application and de highlight the delivery option within Canada and you'll be fine.

News from ICES:

Ivory Coast: due to a recent increase in the number of fraudulent documents originating from this country, applicants who submit credentials from the Ivory Coast will experience further delays due to verification procedures we have put in place.
