9. Certificate of Origin
0. Medical Form
1. Valid Parent Tax Clearance
2. Attestation Letter
3. Online Receipt
4. Online Course Registration Form
5. Matriculation Oath
6. Payment Receipt for Matriculation Gown
Arrange your documents and put them in a file the way it is
been listed above but do not pin them yet, make three (3)
copies each of the listed items. Clearance commences at the at
the Admission Office of each Campus Monday to Friday from
