to you at your college office to fill)
2.Original and photocopy of academic
certificates (Testimonial or original
o.level result if avalable,acceptance
clearance printout,school admission
letter,matric number print out,matric
oath etc).
3.Original and photocopy of birth
certificate.
4.Local Government identification
letter/certificates
5.Original and photocopy of duly
completed medical form (To be done
at school with #7,500 and x-ray of
1,000 anywhere you like).
6.Letter of attestation of good
behaviour.
7.six recent passport photograph.
8.Parent valid tax clearance.
9.Evidence of payment of #500 Osun
state capital project development
levy (To be paid at tax offices in the
State of Osun).
10.JAMB ADMISSION LETTER.
