Improper installation or sometimes something just fails. I've seen it pull from its ball bearing, also seen it fall just because the cross pin/bolt cut. So many things can make it fall.

Always pay attention to yours. If you hear squeaky sound, that could mean the ball bearing is getting bad - anything can happen.
If it wobbles/shakes due to unbalanced blades, that cross pin/bolt fit dey chop. It may cut off over time.

Occasionally, I always climb to inspect the ones I have. Check the blades, I ensure they all still tight. Check the cross pin/bolt/nut if they still in place. After everything I dust am.

My own not fit fall on me. Still running over a decade now. Only replacement of capacitor and regulator in one of them.
