Not all hospitals have emergencies too. If a hospital does not run emergencies patients should not be forced on them.

Some cases are also beyond the hospital expertise. For example some hospitals have emergencies but are not major trauma units, so will be wrong to accept major trauma cases, and leave them to die.

It would be good for Nigerians hospital to have an integrated system where patient can be signposted to the best place depending on each presentation

Patients abscond everywhere else to avoid paying, but hospitals by law are required to accept any critically ill patient at the emergency unit, even it is a criminal. In fact, if a patient who absconded during the last time he was treated shows up again at the same emergency unit with a critical injury and the staff recognize him, the hospital is still mandated to admit him again. It is only in Nigeria that this stupid attitude of rejecting patients happen. Travel out and see for yourself. No wonder the country is in shambles.

Right. But being 'ill-equipped' is not to the extent that they cannot at least administer first aid treatment before making their referral.

No hospital in Benin ,or Togo tries that,their license will be withdrawn,it is against their law.

But in Naija ,hospitals will be rejecting people as if they do selective treatment .looking for those with minor cases of headache, body pain, malaria etc.


Same way Nigeria police behaves, they run away from armed robbers,terrorists, bandits,militants. But will be patrolling looking for yahoo boys, pickpockets, responsible citizens to arrest and extort, even meddling into issues of husband and wife quarrel.

If this is how referrals work in Nigerian hospitals, then, we are really in a huge mess! This is not standard referral. A critically ill patient is brought in by a private citizen without any first aid, and you expect the same private citizen to transport the patient to another hospital in the guise of a "referral?" Is that what you call a referral?

This hospital has an emergency unit. Immediately the patient is brought in, the hospital should accept the patient, apply first aid, make some examination/imaging and documentation, and if they now confirm that the patient needs to be referred to a different hospital, this hospital should PROVIDE transportation (either via an ambulance or whatsoever), accompanied with a doctor's report from the hospital the patient was initially seen. Now, that is a referral.

You don't attend to a patient at all. You don't even know the diagnosis/prognosis, no imaging or examination whatsoever. How do you know it is a case that you cannot handle? How do you know the right hospital to refer him to when he is not even diagnosed yet? How do you expect the private citizen who brought him in (who may be a good samaritan attending to his own private business) to have the resources to transport him to another hospital? No doctor's report showing anything! And you call that a referral!

Too many evil people in Nigeria. Even the nurses are evil. sad The last time I went to a govt hospital, if you see the way they delayed me. As soon as I got the first aid and I was relieved, I ran to a specialist same day in another region. Very evil nurses. And the doctors arent loyal. They arent patient in the govt hospitals because they are rushing to "cut jobs" in private hospitals. Always in hurry. So, you wait endlessly for them cos they are always late, while the nurses keep bullying you. And finally when they majestically arrive, they rush you seriously while the nurses and matrons even bully you more. Very terrible set of humans. They actually made me hate the medical professions. I do not even have anytning inside me to encourage anyone I know to study or practise any medical profession in this nation called Nigeria. If you come from abroad, you are on your own. Better you do not come. I that used to encourage you to visit Naija is telling you to keep off if you value your life. I have told you the truth oooooo. Keep off! Buzz off!! Run away from Nigeria. This is a "useless" nation with useless leaders.

in a case of an emergency a doctor that is clueless on how to attempt to stabilize the patient before referring him or her to a better hospital is an incompetent doctor. nobody is saying they should do magic but at least try, do your bit as someone who has studied medicine. it's definitely safer that way than referring someone that's in a near death situation to another hospital

I can understand it's is not proper from an ethical perspective. But from business perspective. It is.
nothing like perspective here. the same ethics bind both private and government medical professionals. if you are not ready to go by it, don't open a hospital.

Nigerians are just plain evil people (every pun intended). Hospital will refuse access to medical treatment for accident and gunshot victims until they see police reports on the ground that any doctor that treats such patients could lose his licence. Let us be realistic, how many doctors have lost their licences due to treating people from gunshot wound or accident compared to the number of people that lost their lives as a result of this ill-concieved practice? That is how one useless good-for-nothing lady was arguing with me on Twitter that only a foolish doctor will treat such people that I'm ignorant of any medical protocols. Imagine, what protocol is more important than saving people's lives, Nigerians tend to show this vainglorious attitude when they attain one position in life that makes them feel they know better than others. The essence and the primary purpose of setting up any medical facilities is to save lives, any other things or protocols are secondary. Even if the person is an armed robber, treat him and inform police about it, he can later face his judgement with the firing squad. In the course of my argument with this irritating lady, I did a little research and found out that government had even enacted "The Compulsory Treatment and Care for Victims of Gunshot" act since 2017 that makes it compulsory for doctor to attend to victims of gunshot and accident and failure to do so could attract fine and jail time for the doctor. The truth is that, we have laws that protect every right of the citizens in this country but people in charge don't implement the laws or only apply them discriminatively based on the people they know or personal and selfish reasons. Now, I will go back to my first point that Nigeria is a country filled with evil people, only looking for any reason to unleash their evil, police report has now become a demon causing people's lives.

This is exactly why the country is the way it is. The culture of corruption and impunity now transcends every sector. In this case, we are seeing what is a daily occurrence in several hospitals, and, as expected, guilty medics on here are defending the act. But this is not limited to hospitals only. From government high schools where teachers refuse to show up to their classes when they have a class to teach, to colleges where bribery is the order of the day, to police stations, banks, secretariarts, churches, etc. Pretty much every institution is messed up. This is why we are where we are today.

This is why Law remains a lucrative profession here, because people who understand their rights and how these laws apply can easily exploit these loopholes and get tons of people and organizations sued and charged for negligence annually, and make a lot of money through compensation. But again, we live in a country where even our own leaders don't obey court orders, so what is the assurance that you'll get any compensation when you sure? Sigh, I tire for this country o.

Not always the case.

There are times when even things to stabliize a patient are not available. In advanced countries there are hospitals without emergencies, and no one will even look at you if you bring an emergency there, because they do not have the facilities to do so. When it come to emergencies the most trained to stabilise the patient may not even be the Doctor, and that person at that time should take the lead role. It could be a nurse.

The onus is on the government to set up an emergency response service that effectively triages patients, and convey them to the appropriate service. After triage they may be ambulatory and able to walk to the hospital, and then be asked to do so. Others may have severe head injury, and be taken to a facility that has head injury services.

Imagine taking a patient with shortness of breath to a outpatient clinic with no x-ray facility, no ambulatory ECG or oxygen, and you are forcing the doctor to resuscitate them. I have seen this happen a number of times. I have also seen a hospitals with no orthopaedic services splint a patients broken limb, and asked them to go else where and they were still dragging that they must be accepted.

As long as patients airway, breathing, and circulation is fine, and he is not actively bleeding, then he can go to where he would have the most responsible care.
