Is it from same headquarters Abuja or Slot?

I'm very sure that APL is from spot source, if not, there won't be a need to ask if it's original or fake.

ASC cadre needs guarantor's form
And CV
There's no cv format anywhere
The only thing you will be given is the application letter and acceptance letter
That's the only sample of letter you will be given to check to write yours

File number usually comes when documentations have been done & service number allocated. Just as you have hospital card numbers.
There's usually NHQ file number & State, Command or Formation file number
