You don’t understand the concept.

Your example of sickle cell is not appropriate. Sickle cell is an autoimmune disorder. Meaning it happens just by having the gene. In essence it is not triggered. Atrophy of Podocyte is not an autoimmune condition.
The gene is present but recessive. A person would have to be inoculated with tsetse fly parasites for the gene to be activated.
It would damage the Podocyte cell of the kidney in the process of destroying the parasite.
Just the same way cancer surpassing gene control the development of actual cancer in healthy tissue.
