2days after original post, replacement engine was installed and vehicle left workshop. It's been fine ever since.

I asked a few people in the industry and they said it was a rare occurrence, but they had seen whole valves drop into combustion area on this engine.

you were partially correct. The engine had sticky valves. It almost happened again on a different vehicle , but this time we caught it early .

Used a non invasive method (valve cleaner) to fix the problem.

you were partially correct. The engine had sticky valves. It almost happened again on a different vehicle , but this time we caught it early .

Used a non invasive method (valve cleaner) to fix the problem.

This is typical case of a back fire. This went straight back to the engine hence the occurrence of this. I once dealt with this. Exhaust becomes red as hell fire after 6secs of starting the vehicle. This might be an exception but it's a back fire. Pele sir
