Take a look at civilians casualties as a result of US strikes during their active participation in the middle east war on terror, which happens to be a lot (hundreds of thousands)
Most of it are either deliberate willing targeting, poor surveillance or rarely malfunction ordinance.

NAF is plagued with asset issues, we don't have enough surveillance platform for the vast northern territory, and worst you're trying to look for bandits that's blended into an indigenous community with little/no local informants.
