I am trying to obtain a fresh passport, I saw a DIY thread and decided to do mine.
Please I’m stuck at a particular point, so I attached the screenshot.

Please gurus, how do I get those documents and upload to complete process

How u take know

1) You can get a birth certificate from a government owned health center. Most especially those heavily involved in maternity, with about 2-3k you can get an original backdated NPC birth certificate

2) Go to your local government of origin, and they will issue you a local government of origin certificate for about 6 - 10k. But recently, some LG made it a must for you to present a signed recommendation letter from your community of origin when requesting a local government of origin certificate. I was denied on 2 attempts by my LG chairman because of that worthless piece of paper.

3)Does that apply to you?

4)Your original NIN slip should suffice.
