My wife is 5 weeks and 5 days pregnant (last period Aug 14) and blood test and urine has confirmed pregnancy.
Since last week Wednesday we have been noticing light spotting occasionally. Sometimes we see tiny clots of blood in her deejay.

We were at the hospital today and paid a lot of money for evaluation.

Doctor says :
1. he has confirmed that no signs of ectopic pregnancy (tubal) .
2. He however said he is unable to see a gestational sac in the uterus. He used a small scanner that I personally doubt its efficacy.
3. He said the only other thing that can cause a non visible gestational sac is a complete miscarriage since he is sure there is no ectopic pregnancy.

Now he has referred us to another laboratory for a scan on Saturday (pregnancy will be 6 weeks and 4 days at the time.)

My question is,

1. shouldn't he be doing a transvaginal scan instead of a pelvic scan as that is more accurate ?
2. Is referring us to another lab a subtle admittance that their equipment is not adequate ?
3. Is it possible to have a complete miscarriage and just experience light spotting like my wife is having? She only notices it when she wipes and even the doc said what he saw during examination is not a cause for concern.

Please advise me as I am really loosing my mind here.

Well, congratulations to you both.

To your questions
1. Yeah, a transvaginal USS is more sensitive at that gestational age, however, a trans abdominal should be able to pick some endometrial changes and tiny gestational sac at this point.

2. I figure his referral is rather for second opinion seeking
3. Possible because at this gestational age, the fetal parts/poles and nodes may not be properly formed so she may not pass a lot of fleshy material. Still a tiny embryo but I think she may just be having an implantation bleed.

Is she experiencing lower abdominal cramp with the bleeds?

My suggestion, give it one more week or two and repeat the USS, everything should be clearer by then. Either a failed or establishment pregnancy.

At 6 weeks he should be able to detect the child‘s heartbeat as far as I remember. Please go and see another doctor.

Hi baba baby, don't over stress yourself. With the kind of equipment we mostly have in our hospitals,I believe 6weeks is too early to see the pregnancy through an abdominal scan. Biko give the other lab another two weeks before going so they could see your little bean clearly.

I know how long the wait would seem but you honestly would be putting your mind at rest by giving it more time. And if really the pregnancy isn't viable, that time will tell too. Rooting for your little champion daddy and mommy.

It's hard to wait so we will go and do a trans-vaginal in Saturday (6 weeks 4 days) at a standard diagnostic center with better equipment , I am sure we will get result.

Thank you so much for your time, the doc sent us to do another pelvic USS on Saturday (6 weeks 4 days) but we will be doing a trans-veejay instead.

This is to give an update.
We were at diagnostic center and did a trans-veejay today.

The sonographer said... he is unable to see a gestational sac, he noticed however that the endometrium is significantly thickened.

He also said no signs of ectopic pregnancy.

And miscarriage is also a doubt cos most miscarriages will still show gestational sac except its complete miscarriage of which the bleeding will be heavier and significantly more noticeable.
Is it possible to have a complete miscarriage and not have any signs of it?

He asked again if we had a positive pregnancy test and we told him we have tested thrice in different places and result is positive.

His suggestion is that the pregnancy might be younger than we think, even if my wife last menses was Aug 14 he feels if she ovulated much later pregnancy might be younger and so making it difficult to see a gestational sac even with Trans-veejay.

Now we asked him what could be causing th blood clot, he said there is no sign of ectopic pregnancy same thing the gynecologist said he thinks the clot may be due to changes in the uterus or even from fibroid(my wife has small fibroids) .

The conclusion is that we wait for 2 more weeks and come back for a scan.

Any inputs is welcome.

Oga, stop wasting money. Pregnancy doesn't hide. If it's there, it will show in a few months. As long as there is no sign of ectopic Pregnancy, that means she is safe and not in any danger. Alot of pregnancies self terminate naturally and its nobody's fault. That's just a fact of life but the one that will stay will stay.

Turns out it was ectopic (ovarian).
She went in for a double salpingectomy and open myomectomy (submucosal).

Took her 3 months to heal.

She got pregnant naturally at the 2nd try/month after recovery even with one fallopian tube.

Let them check the adnexea/ovary it could be that the fetus may be fused with the ovary.

Please keep me updated.
