Ayatollah is not a democratically contested position.

It’s both a religious and traditional leadership position.
Iran operates both a form of democracy ( They have a president) and a form of Islamic theocracy, which places the Ayatollah above the president in Iran.
So ultimately, the Ayatollah makes key military, economic and diplomatic decisions.
