In the a absence of a picture it will be hard to say for certain. Also your description of his symptoms was very vague at best. An infection in the penile shaft could be caused by different things. First things first is he circumcised ? What other symptoms do accompany your definition of infection? Redness? Itching? Pain? Swellings? Rashes? Pus? Etc..

Penile shaft infection can be caused by poor hygiene and or a fungus particularly candidiasis, but so too other bacteria infections like staph, viruses like herpes and other STI..

If Nixoderm and Miconazole cream didn't offer some relief then it's likely not a fungus infection. At this point your best bet is to take your brother to a hospital for appropriate follow-up. A physical exam will narrow down the cause and so too blood tests will either rule out or confirm ..
