There are a ton of enterprise-grade CBT software that I'm surprised people still come to ask.

With some creativity, you can use Moodle's exam module for this. For pure CBT software:

ExamsSys (formerly Rogo) from the University of Nottingham:

My old reliable, TCEXAM: hxxps://tcexam[.]org (been using this on on-and-off since like 2010)

These three are my defacto preferences. Moodle, ILIAS and Canvas LMS or any other LMS would work. For a secure-locked down environment, I often pair Moodle with SafeExamBrowser (hxxps://safeexambrowser[.]org/ ). Best if it will be in your exam centre but I've also used it in one instance for an exam with BYOD. Extra planning ahead is required if you will be using BYOD.

Just came across Quiz/Survey/Test (hxxps://qstonline[.]ca/) downloadable at (hxxps://sourceforge[.]net/projects/qstonline/files/). Looks interesting, especially based of a reddit comment I read

QST is used globally (from individuals to businesses, K-12 schools, colleges, universities, etc.) and also converts questions used by other LMS's (Canvas, Moodle, etc.) to QST XML and vice-versa.

I came accross a website called CBT Hub cbthub[dot]com[dot]ng that allows you create and manage examinations online. It was super easy to use, and the support is great.

Why not create a custom solution which you can host yourself and even sell as a service
