I have a theory. It might sound stupid but still. I think he has a parasitic organism inside him that cannot survive in a live host. So for the parasite to live, it has to ensure it's host is dead, it has to control its hosts brain and thinking, maybe make him feel like he's been choked to influence his decisions of cutting himself. To relief the pressure around his neck. You can see how relieved he was after cutting his neck, when the blood was pumping out, like a noose was just removed from his neck. Before he realizes what he has done, it is already too late and then his body dies. The parasite then gets to feed on his body, maybe for months until there's nothing left.
