In fact physician is the right term as the doctor was initially used to appreciate the efforts of young "doctors".The word doctor is actually academic title , to teach in which the bearer has to be a PhD older.the older doctors then in the UK would prefer Mr over Dr because it was a title to honor general practitioners or new physicians

A medical intern is not a doctor in making, they are qualified doctors with licence to practise. Licence though is provisional, not full. They get full license after completion of internship.
So, as per the picture above, lady A is the doctor.
