A medical intern is not a doctor in making, they are qualified doctors with licence to practise. Licence though is provisional, not full. They get full license after completion of internship.
So, as per the picture above, lady A is the doctor.

The lady is an house office, an intern.
Regardless of possibly being fresh from school, she makes the final decision of all 3 and will put down plan for others to carry out.
Sure, she will also revert back to her seniors to give feedback who will then review the patient again if necessary.
