This is obviously a case of gynaecomastia which results from an imbalance of the female hormone; estrogen. He has higher than normal estrogen which explains the development of breasts.

It is called gynaecomastia

It's a health issue caused by hormonal changes, medication, drug abuse, disorders, and etc. He's not trans.

I honestly couldn't care less about his sexual orientation or gender type. That is not my area of specialty.

The other is a case of actual growth of breast tissue.

Only surgery can fix that case.

This man must have case 2 and is caused by hormonal imbalance.

Mainly estrogen being more than testosterone in his body.

He will be experiencing low libido, mental fog, low energy amongst other symptoms.

Apart from getting surgery he needs to fix the underlying issues simplest of which is fixing vitamin D deficiency. He must be someone who stays indoors without access to direct morning sunlight.
