Mastectomy needed. Drug therapy won't work here. See a good plastic surgeon for an end result that is aesthetically pleasing but first you must rule out Disorders of Sex Development (DSD). Visit an endocrinologist to ensure that this is not XX DSD

It does look like he's past puberty and should seek medical intervention, I don't think his case needs plastic surgery as if you don't address the hormonal causes.

Many thanks for the kind comment. Recommended mastectomy because the breast development is far too advanced for selective estrogen receptor modulator class of drugs to work in a genetic male. Whatever the diagnosis is (besides XX DSD), surgery is most likely indicated.
