Not all of them though...

It was actually caused by those maintaining old abandoned phone numbers with their banks such that the abandoned lines got credited each time they tried recharging their current lines. Their defence had been that they never applied ab initio so any dispute arising from the service would place the banks at disadvantage, hence the need for some of the banks to introduce some sort of application.
