Statement is one of the things to be submitted,so I advise u wait till u can have it,however,if u can't, u should go ahead and submit whatever u have.
Pls and pls,ask others for advice too;cos I'm not an expert.

Yes u will still obtain a form from UI,I don't know the amount it is sold,UI does not conduct any exam.
Pls and pls,subject all my answers to confirmation as this is based on my rough knowledge.
