2 Passport photograph..
And most importantly Handwritt3n Application L3tt3r.

And pls mak3 sur3 you writ3 your PHON3 NUMB3R b3low ur addr3ss in ur Application L3tt3r.

So u will have to change the state to ur own individual states and individual P. M. B.

The Passport Photograph is OPTIONAL!
You may or may not include it.

I'm not including it. So it is left to u how u wanna package it should u want to include it.

Pack your docs in a brown 3nv3lop3,s3al3d with ur nam3 and phon3 numb3r writt3n on it. Submit it to ur DSS Stat3 command. Go back hom3al and pr3par3 for the 3xam.

It is not compulsory u submit by urs3lf. If you ar3 domicil3d far from ur stat3 of origin, u can g3t a smart r3lativ3 to h3lp u d3liv3r th3 package to DSS offic3.
