Video was most likely recorded before he got transported to the hospital.

The story says that d hospital has a presidential suite. This is because it is a military hospital designated for treating American presidents. Which means, if d president is healthy enough but not discharged, he can attend to national issues from d hospital.

So I don't see what wearing cufflinks and dress shirts have got to do with negating where d video was shot

A war in one insignificant country at a far corner of the world can trigger a global war that affects everybody.
