Personally I've done work in US foreign military sales in the design and construction of such structures. One of the more significant problems is vibration isolation and shock attenuation (VISA) where one might expect to be deep enough but not have adequate mechanical isolation from the blast to avoid significant damage. In the US inventory we've had a variety of very large weapons to be delivered in lay-down mode where the weapon is dropped, parachute retarded, and literally rests on the surface before detonating. Smaller weapon yields can be employed when those weapons have a very low circular error probability and are designed to penetrate the earth before detonating. The warheads may be rocket propelled to increase their velocity and made of materials that are quite hard and survivable. To get a general idea of same, albeit in a non-nuclear form, take a look at the BLU-107 Durandal.

My own experience, I was a program manager for building dispersed and hardened structures for a foreign Air Force. They were duplicated and dispersed 5X, buried 60 feet underground using hardened construction. Other countries go to even greater lengths, thus necessitating the nuke option. Would you tip your hand in clearing the route? Sure, but what will they do? These aren't things that can just be relocated. For an adversary who is at a numerical and technological disadvantage...time is the real enemy because you'll eventually run out of it.

As you can see, this information and much like it is available to them if they wanted it to be bomb proof. It wasn't meant to be.
