You've explained it better than most people I've spoken with. So basically the system is designed in a way that makes continuity almost impossible — each hospital starts from scratch by default. Has this ever caused a real problem for you personally or someone you know? Like a situation where starting from scratch led to a wrong call or unnecessary cost?

Unfortunately there is no interconnectivity between hospitals especially private hospitals.Each patient who presents at a new hospital is starting(or almost) starting a fresh.
It's a question of cost implications.

When you go to a new hospital,they are seeing you for the first time.They don't know you or your past medical history.They will need to make a diagnosis based on the test you carry out and the results they get.That will determine their treatment plan for you.
Usually when you are referred from another hospital,the referral letter may not contain(usually doesn't) all the information they need.Your records from the referring hospital too is usually not sent to the new hospital.The new hospital therefore has to take you through their own examinations and tests.
