Pls pals must one scan the form DS 230 and DSP 122 before sending the hard copy to the Consulate? Or is it optional?

Pals pls, form DS-230 part 1 requests for permanent address where i intend to live in the united states including the name of the person who currently lives there. Does it mean i will live with the persons name i give that lives there? Or hw? I hav a distance relation in califonia whose name and adress i intend to give. Pls whats the ímplication? Must i live in her house on reachin the states?

Pls folks. Am i allowed to combine two ssce results taken in two different schools in 2009 and 2011. The second result has everything except chemistry. Pls what do i do?
