Pathological lying ( also known as pseudologia fantastica or mythomania is not a standalone diagnosis but a symptom of deeper psychological, neurological, and developmental issues. Its root causes are multifaceted:

Underlying Mental Health Conditions: It frequently co-occurs with personality disorders, particularly Antisocial Personality Disorder (ASPD), where lying serves manipulative or exploitative goals, and Narcissistic Personality Disorder (NPD), where fabrication maintains an inflated self-image. It also appears in Borderline Personality Disorder (BPD) as a response to abandonment fears, ADHD as impulsive behavior, and Factitious Disorder (Munchausen syndrome), where illness is fabricated for attention.
Neurobiological Factors: Neuroimaging studies reveal structural differences in the brains of pathological liars, specifically increased prefrontal white matter (linked to verbal and cognitive flexibility) and decreased prefrontal gray matter (linked to impulse control and moral reasoning). This suggests a biological deficit in inhibiting dishonest impulses and regulating socioaffective responses.

Developmental and Environmental Trauma: Many pathological liars develop this behavior as a survival mechanism in childhood environments marked by abuse, neglect, or severe invalidation. Lying becomes an automatic defense against perceived threat, shame, or rejection.

Psychological Deficits: Chronic low self-esteem and a fragile sense of self drive individuals to construct a fictional persona to gain acceptance or avoid feeling inadequate. Some researchers note that pathological liars may engage in significant self-deception, blurring the line between lying and delusion because they partially believe their own elaborate fabrications.
