Please i have a serious issue.

I have been trying to get a transcript from my secondary school but they have no idea how to get it done. They are requesting that i bring a sample of what the transcript should look like. Please Nairalanders i need help!!! I would appreciate if i can get samples of secondary school transcript here. I need to send it along with my documents for admission consideration abroad. Help!!!
