you will find the accommodation form when you log into the eportal and click on report but if you are using a pc the let you mouse be on report then you will see it,sorry for the late reply

One For Self After Duly Signed By Hall Warden) not really sure but i think thats all you need might be wrong tho just asks others and keep your eye open to what people are doing in school and cyber cafes in campuss on the 28th of may
