In addition to what vision 2050 has posted, have the following documents before coming to purchase the form so you don't come two times. you have to submit 1 copy each after filling the form;

1)3 Passport photographs( bring like 5 in case)
2)Primary 6 cert.
3)SSCE cert
4)1st degree results, professional certs, HND, ND, PGD, MSc etc
5)Birth cert
6)CV
7)Cert of local govt of origin
8NYSC discharge cert or Exemption cert
9)Evidence of payment of the appointment form


-Submit the complete forms immediately or latest the next day of collection
