Rename your passport, ensure no quote within name. The issue lies within the bold text. The programmer's fault, not enough filtering. So much for not using PDO.

TRY USING A GOOGLE CHROME ON A WORKING DAY, MAKE SURE YOU HAVE A GOOD 3G NETWORK.
CONVERT ALL DOCUMENTS TO JPEG THAT IS LESS THAN 50KB USING PAINT ON YOUR COMPUTER.
IT SHOULD WORK FINE
